Claim Reporting Tips

VLCT PACIF has a staff of six claims professionals whose sole job is to handle the property and casualty claims of our municipal members. In order to aid the claims professionals in providing the most expert claim service available, please follow the claim filing tips below.

When filing any claim it is important that you contact the VLCT PACIF Claim Department as soon as you can. The sooner an investigation can begin, the faster the claim can be resolved. To contact the claim office, call 800-649-7915 or e-mail Trisha Clark, Administrative Assistant, Claims.

When filling out the claim report form it is very important to include as much information about the incident as you can. The following information can all be relevant to a claim:

  • Names, addresses, and phone numbers of the parties involved
  • When and where (exact location) the incident occurred
  • Witness names, addresses and phone numbers are crucial to the investigation of the incident
  • Were there any injuries
  • Where did the parties go for medical treatment
  • Were the police contacted

As a general rule of thumb, there can never be too much information on a claim reporting form. Also keep in mind that when reporting a workers’ compensation claim, time is especially critical. As the employer, by law you have to file a First Report of Injury within 72 hours of receiving notice or knowledge of an injury that occurred to an employee while he or she was in the course of his or her employment.

As mentioned above, if you ever have any questions about filing a claim, please call the claim office at 800-649-7915 or contact Trisha Clark. To further facilitate filing a claim with VLCT PACIF, you may file the workers' compensation First Report of Injury online in our Customer Service section. Other PACIF claims forms (liability, property and automobile) are located in the Customer Service section as well, where you may print them for use as needed.
