Members that offer VLCT’s vision coverage should note that EyeMed has stopped accepting the EyeMed enrollment/change form by fax or email. Primary enrollment or eligibility updates and changes must now be made directly through the EyeManage portal. The EyeManage Client Portal Overview is the user guide to consult, and it is posted on our Vision Insurance Guidance and Instructions webpage.
Please contact Kelley Avery, VLCT’s Senior Benefit Programs Administrator, if you require access to the EyeManage system. In the meantime, Vision program members may submit their EyeMed enrollment change forms through Kelley, by fax to 802-229-2211 or by email to firstname.lastname@example.org, and she will process them.
